During the French Revolution, after seeing the speed with which the carriages of the French flying artillery maneuvered across the battlefields, French military surgeon Dominique Jean Larrey applied the idea of Ambulances, or "Flying Carriages", for rapid transport of wounded soldiers to a central place where medical care was more accessible and effective. Larrey manned Ambulances with trained crews of drivers, corpsmen and litter-bearers and had them bring the wounded to centralized field hospitals, effectively creating a forerunner of the modern MASH units. Dominique Jean Larrey is sometimes called the father of Emergency Medicine for his strategies during the French wars. Emergency Medicine (EM) as a medical specialty is relatively young. Prior to the 1960s and 70's, hospital "emergency rooms" were generally staffed by physicians on staff at the hospital on a rotating basis, among them general surgeons, internists, psychiatrists, and dermatologists. Physicians in training (interns and residents), foreign medical graduates and sometimes nurses also staffed the ED. EM was born as a specialty in order to fill the time commitment required by physicians on staff to work in the increasingly chaotic emergency departments (EDs) of the time. During this period, groups of physicians began to emerge who had left their respective practices in order to devote their work completely to the ED. The first of such groups was headed by Dr. James DeWitt Mills who, along with four associate physicians at Alexandria Hospital, VA established 24/7 year round emergency care which became known as the "Alexandria Plan". Soon, the problem of the "ER", propagated by published reports and media coverage of the poor state of affairs for emergency medical care had culminated with the establishment of the first emergency medicine training program at Cincinnati General Hospital, with Bruce Janiak, M.D. being the first emergency medicine resident in 1970. During the 1970s, several other residency programs developed throughout the country. At this time, EM was not yet a recognized specialty and hence had no primary board certification exam. It was not until the establishment of ACEP, the recognition of emergency medicine training programs by the AMA and the AOA, and in 1979 a historical vote by the American Board of Medical Specialties that EM became a recognized medical specialty. Assuming that this wasn't written by someone named John Carter, MD, I'll take it as evidence that County's ER may well have been founded in past half century. We know from "Good Luck, Ruth Johnson" that County is over 100 years old, so the ER would have to be a more recent addition to work. |

I seem to recall that the Alan Alda character was introduced as someone who had been a pioneer specialist in Emergency Medicine, though not at County. That would make sense if the specialty arose in the 1960-70s. Maybe Alda's character was modeled on the historical Dr. Mills. I think Moretti's speech about the connection between battlefield treatment and civilian ERs took a good deal of artistic license with the facts. There are some similarities between the two types of medical care, but major differences as well: the emphasis in the military has been on getting mobile aid to the battlefield (MASH), so logistics is a much bigger issue than in civilian care; the injuries treated in war are almost always caused by gunshots and explosives, while civilian ERs see a range of diseases and injuries, etc. If you're interested in this subject, I recommend Attul Gawanda's book "Better," which has a chapter on medical care during the battle of Fallujah, and there have been some longer books published recently on battlefield medicine in Iraq. At any rate, it was an exaggeration for Moretti to say "we wouldn't be here if it wasn't for war." If I were writing the finale, they would find an elderly gentleman who had been sitting in the waiting room for 15 years without ever getting seen. Then he would die. That would be more like some of the public hospitals in my area! |

I remeber the old show "Emergency!" which made a big deal about paramedics and new-fangled ERs--that was late 60s, early 70s. It would make sense if
they got someone who started the Emergency Medicine specialist training at County. (Please, not a Carter, though!)

I think it's great news for Parminder and I do think Neela will end the show with Ray because, you know, it's ER and all, the message is that people
without legs can have a happy end and so.
If I could ask only one question to one ER cast member, past or present, I swear that I would ask Noah Wyle why on Earth he wants so much to be part of the end of ER. I understand it was part of his life and such, but hellooooo, people move on and so. Oh, I totally think we should set the biggest chat ever right after the episode. I mean, how many people can say they've endured 15 years of a TV show and God knows how many of cyber friends and cyber frenemies? Teeeeheeee, really that would be great! Iczeeeer, let's put that chat room to use! |

Eh.....ER was a huge part of his career start as well as the means for his current fortune. He started with it, why wouldn't he want to finish it?? I

This is not exactly an ER spoiler, but it appears that all 10 p.m. drama will soon come to an end on NBC, to be replaced by Jay Leno.

The Leno thing just shows how creatively bankrupt the networks are. There has not been a 10 o'clock show introduced in the last five years that has become
a breakout hit. However, I also think late night viewing has a certain audience it appeals to and that people are just not going to watch it at 10.

hes lucky he has one memorable huge credit i cant blame him for trying to milk it. my ex went from acting in Broadway as a lead to doing theater now in
kentucky. And I have been 11 years struggling and the closer I have been is a network audition call back and lots of OFF BDAY

Clooney really IS a star now, and there are probably more people who think of ER as a show Clooney was in than people who think of Clooney as an actor who was in ER. Wyle is always going to be an actor who used to be in ER; he will never approach Clooney's status. |

I don't have a problem with NW coming back for a final curtain call, but his attachment to that particular fantasy ending wherein Carter takes over the ER
and puts his feet up on the admit desk does seem a tad obsessive. He's been talking about it since he re-upped his contract at the end of S7.
